OCFS forms are the official documents the New York State Office of Children and Family Services requires from licensed and registered child care providers — applications, background checks, health and safety records, and enrollment paperwork. Anyone running a family day care home, group family day care home, day care center or school-age program in New York needs them. Every form is published free on ocfs.ny.gov, and this page indexes the recurring ones by number so you can find each one.

What are OCFS forms?
OCFS forms are the numbered documents the New York State Office of Children and Family Services uses to license, register and monitor child care programs — each one carries an OCFS number, a revision date, and instructions telling you where it goes. They are published by the State, free to download, and the copy on ocfs.ny.gov is the only version your licensor is guaranteed to accept. Third-party copies go stale when OCFS revises a form, which it does on its own schedule.
Which OCFS forms do I need to open a daycare in New York?
More than the ones below — so start here, but do not stop here. Where do I download OCFS forms as PDFs?
From ocfs.ny.gov, which publishes every form as a free PDF — that is the authoritative source, and the links in the table above point to it rather than to copies hosted here. The State's forms index lets you filter by topic and search by form number when you already know it.

The provider completes the licensing paperwork and sends it to the State; parents complete the enrollment paperwork and send it to you. That split matters, because the two halves have completely different lifecycles — you file a licensing application once and renew it on a cycle, but you collect an enrollment packet, a medical statement and emergency contacts from every new family, forever. New York publishes its own enrollment form for this: LDSS-0792, Day Care Enrollment, which the parent or guardian completes at enrollment.
The licensing half stays exactly where it is: OCFS forms, downloaded from the State, submitted through the State's channels. Nothing about that changes and nothing about it runs through us.
The parent-facing half is the part that repeats, and that is the part we handle — one link families complete on a phone, covering child information, emergency contacts, authorized pickup, immunization records, allergies and consents, with a checklist per child showing what is still missing.
Do OCFS forms apply in New York City?
Partly — it depends on the kind of program you run, and this is the detail that catches City providers out. Child care centers in the five boroughs are permitted by the NYC Department of Health and Mental Hygiene under Health Code Article 47, which requires a permit issued by the Commissioner and brings its own City forms and inspections.
Home-based programs are different. Article 47 explicitly excludes “a group family or family day care home, or school age child care program” from its definition of a child care program, naming them as State-regulated — so if you run family or group family day care in New York City, you are on OCFS regulations and OCFS forms.

Common questions
I run a family day care home in Brooklyn — is that OCFS or the City?
OCFS. New York City Health Code Article 47, which is what creates the City’s child care permit, explicitly does not cover “a group family or family day care home, or school age child care program” — it names them as State-regulated. So home-based family and group family day care in the five boroughs runs under OCFS regulations and OCFS forms, even though inspections in the City may be carried out locally.

Check your own requirements with OCFS or your licensor before you retire any paper file, and treat anything collected online as an addition to that until you have confirmed it.
What if a form number on this page is out of date?
Always download the form itself from ocfs.ny.gov rather than from any third-party copy, including ours. OCFS revises forms and retires numbers on its own schedule, and a PDF saved from a blog post a year ago may no longer be the version your licensor accepts. This page carries a visible last-updated date so you can see how fresh it is.
